Personal & Clinical Information
In accordance with legal and professional requirements, personal and clinical information will be kept confidential and secure for at least 8 years from the date of last treatment (8 years from their 18th birthday in the case of children, and includes personal, social and medical information, records of Physiotherapy assessment (including photographs and video recordings), advice and treatment, reports and correspondence created by Ian J. Johnston trading as achievephysio, or his representatives, and reports and information from other parties involved in your treatment or care (potentially including your GP, hospital consultant, or other healthcare professional), and also your insurers, or employer or other organisations arranging and/or funding your care and their agents (potentially including solicitors) in paper, electronic or other data storage form.
Your condition, Ian J. Johnston trading as achievephysio’s (or his representatives) assessment, treatment and outcome, and/or your medical details may be discussed with and/or reported to other health professionals including your GP and/or Consultant, verbally, electronically or in writing.
In the case of treatment relating to a road traffic accident or personal injury insurance claim, or medical insurance funded treatment, or employer funded treatment, your condition, all assessments, treatment undertaken by Ian J. Details including your name, date of birth, contact details, nature of your problem, number of appointments, treatment outcome, and how you found out about the practice will be recorded for the purpose of practice records, and monitoring/audit/assessment and reporting of performance, and business development. Your data relating to card payments made to achievephysio will be processed, transmitted and stored by the card payment service provider together with your name, email address or telephone number which are used for receipt issuing purposes. Your name, date of birth, and email address will be entered onto the system of the exercise programme software provider for the provision of home exercise programmes. The personal and contact details of any person accompanying you to a face to face appointment will also be recorded with their consent in case of the need for a Covid-19 related contact-tracing exercise or similar. Neither your personal and clinical information or the personal and contact information of any persons accompanying you to a face to face appointment will be otherwise shared or sold unless as a requirement of law, or Covid-19 related ‘contact-tracing’ exercise.
